Authorization to Open or Change A Bank Account

This form is included in the troop forms packet and is used when a bank account does not already exist for the troop or when the names of signers change. Complete the form including the name and address of the bank, the names of persons who will sign on the account and the name and address of the person who will receive the monthly statements. Submit to the membership & marketing executive who will generate a memo. The memo authorizes the opening of a new account or the changing of the signers as needed. Allow three working days for the memo to be produced once submitted. Refer to Troop/Group Policies. This form becomes a part of the council's troop file and is required for the Troop File Audit.

Bus Transportation Agreement

This form is available from the service unit manager and is used when a troop/group is using bus transportation for any activity. Refer to Insurance Procedures.

Camp Reservation for Camp Whispering Pines & McFadden Cabin (now available online)

This form is available from the service unit manager to reserve one of the above properties. Complete in duplicate and submit to service unit manager for signature before submitting to the council. A current troop budget must be on file at the Council Service Center. (Cabin is for day use only.) Detailed procedures on reservations and use of facilities are provided in outdoor training courses.

Data Change Form

This form is used to change personal information for both girl and adult member information (i.e. name/address/phone number, etc. changes). Also used to transfer girl/adult member information to a different troop as well as update adult member position codes.

Equipment & Program Kit Request Form

This form is to expedite program kits and other equipment (such as flags, etc.) being checked out and returned to the Council Service Center.

Event Application

Is your troop or service unit planning an event? A new stream-lined (one page) Event Application form has been created, and should be completed and submitted for approval to your membership and marketing executive approximately 4 weeks before the event. This form can also serve as a publicity request form for your event.

Event Participation Form

This participation report is a summary form to be completed by the Service Unit Manager (or designee) and turned into your Membership and Marketing executive no later than four weeks after your service unit event.

Event Registration Form

This form is included in the Program Planning Calendar and is also available from the service unit manager. It is used to register for program activities. Side one is used by troops; individual girls use side two.

Financial Assistance Application

This form is available from the service unit manager and is to be used when a registered girl needs financial assistance. Funds can help to pay for day camp, resident camp, Destinations or council program fees. The parent/guardian completes the form and submits it to the program development director before the deadline. Money is awarded on a first come, first served basis as long as the funds remain available.

High Risk Activity Application

This form is designed to provide troops/groups an opportunity to engage in horseback riding, caving, rock climbing, challenge courses or any other activity not listed in Safety-Wise activity checkpoints. All applications must secure permission from authorized Council personnel before planning or engaging in any high risk activities. Allow six weeks for approval.

Incident/Accident Report

This form is included in the troop forms packet and should be kept with the troop's first aid kit. It is used to report either an incident or an accident such as any accident involving injury that required medical attention or could require medical attention at another time; suspicion of abuse of any nature whether personally witnessed or not; any incident that could negatively impact the child's psyche; unresolved conflicts with other Girl Scout adults; inappropriate statements that are harmful to girls, violation of Safety-Wise principles witnessed. When in doubt if the form is required, fill it out and submit it. Submit "Incident/Accident" form to the Council Service Center within twenty-four (24) hours of the incident/accident. Inform the service unit manager or membership and marketing executive as soon as possible. If an insurance claim is to be filed as a result of this incident/accident, a claim form can be obtained from the service unit manager or membership and marketing executive.

Looking Ahead Form

This form is available from the service unit manager and is completed by the troop leader in early spring. It is used to plan for the upcoming year.

Parent/Guardian Permission for Troop Outings/Health History

This form is included in the troop forms packet and is to be kept with the troop to help establish parental support. It is completed and signed at the parents' first meeting or first troop meeting and kept with the troop at all times. It is important that the health history portion is complete and up-to-date because it may be necessary to provide it to medical professionals. Written parent/guardian permission for each girl's participation must be secured any time the troop participates in an activity away from the regular meeting place and/or at a time different from the regular meeting day and time.

Parent/Guardian Support Agreement

This form is included in the troop forms packet and is used to establish support from parents at the start of the troop year.

Publicity Request Form

This form is used to publicize Girl Scout events and activities via newspaper, TV, radio, and increase visibility in the local community. Submit the form two weeks in advance of event to the Council's communications director.

Rented Vehicle Checklist

This form is required and must be completed and submitted to your Membership and Marketing Executive four weeks prior to your troop's event and accompany any other necessary paperwork required for your outing.

Training Registration Form

This form is included in the Leader Resource Manual and is to be used to register for all council training courses and enrichment classes.

Troop Budget/Finance Report

This form is included in the troop forms packet. It is initially submitted to the service unit manager after planning a year's activities with the girls. It must be submitted no later than six weeks after the troop meetings begin and updated as needed. Complete the finance report section within six weeks after the budgeted year or if change of leadership occurs. Use as a worksheet to project budget for next year. The "Troop Budget/Finance Report" form consists of three report columns - one for the past year's projected budget, one for the actual finance report for the budgeted year, and one for the proposed budget for the coming year. This will result in the same form being used to submit both the proposed budget and the finance report at different times of the year. This form should be filed in duplicate. This form becomes a part of the council's troop file and is required for the Troop File Audit.

Troop Committee Sign-Up Sheets

This form is included in the troop forms packet and is used to enlist help from parents.

Troop/Group Outing Notification & Community Service Log

This form is included in the troop forms packet. It is completed as the troop/group's activity plan is developed. Individual service units have their own procedures for who holds and signs off on these forms. Check with your service unit manager or membership and marketing executive for details. This form must be kept updated, as new information is available. The form must be initialed before the troop can do the activity. The community service portion of this form should be totaled before June 15 when new forms are issued to returning troops. This form becomes a part of the Council's troop file and is required for the Troop File Audit.

Troop Money Earning Application

This form is available from the service unit manger and is used when additional funds are needed for troop activities. Troops may not sell retail or wholesale products of any kind except Girl Scout Cookies, nuts and candy, and Girl Scout Calendars. Other activities such as garage sales, bake sales or items that they make themselves are acceptable money earning activities.

Troop Partnership Agreement

This form is included in the troop forms packet and is used to record an agreement between a troop and a partner who will provide something for the troop. The troop section is completed by the troop then presented to the partner. Once the partner has completed their portion of the form and returns it to the troop leader, the troop leader sends the form to the Service Center. The council president signs and returns the agreement with a cover letter to the partner. A signed copy is sent to the troop leader.

Troop Trip Application

This form is used for trips of three nights or more. It is available from the service unit manager. This form is also used to request sickness and accident insurance for the trip. Activities for more than two nights are not covered in the Basic Accident Insurance. There is a space for computing insurance premiums on the Troop Trip Application. The troop should purchase sickness and accident insurance for the activity. The form should be submitted to the council at least 6 weeks in advance. Please submit a troop check to cover insurance premiums with the application. Refer to Insurance Procedures.

Volunteer Application

This form is included in the volunteer orientation packet and is completed by candidates for a volunteer position. All volunteers for an operational position must submit an application with references and signed authorization for appropriate screening levels. Selection is made following satisfactory review of qualifications and completion of the screening process.

Volunteer Expectations/Performance Appraisal Worksheet

This form is reviewed as a part of the orientation conference. Once completed and signed by the leader and the service unit manager, it becomes a part of each leader's permanent file. The Expectations portion is completed when appointment takes place. The Self-Appraisal portion is completed at the time of re-appointment each year.
